When providing a reference, you must follow the questionnaire format below; references that deviate from this format will not be accepted. You are encouraged to provide thorough responses to support the review of the applicant’s professional qualifications, character, and proposed residency project. If you are missing information about the residency project, please contact the applicant directly.
Your answers to this questionnaire cannot be longer than two pages total (not including cover page). Please note that you must sign this document and save it as a PDF before returning it to the applicant. Note that you should be prepared to be contacted should the candidate advance to the interview stage.
